John Terry has sent an emotional message to Chelsea fans on his way to Stamford Bridge as he prepares to bid an emotional farewell to the Blues supporters.
It is fitting that Terry will lift his FIFTH Premier League title as he plays his last game for the west London club after announcing he will be leaving at the end of the season.
The 36-year-old first joined Chelsea when he was 14 back in 1995 and has gone on to play 716 times winning 15 major trophies.

Terry says it will be hard for him to fight back the tears at the final whistle but will never forget the support he has received over the last 22-years.
The experienced defender went out of his way to record a video on Chelsea fan site cfcuk Fanzine and knows this is going to be a day he will never forget.
Terry said: “All the Chelsea fans – this is going to be a really emotional day for me.
“I just want to thank you from the bottom of my hear for everything you have done for me over the years with your support.
“You have picked me up when I have been down with the disappointments. It is down to you that I have picked myself up and gone again on the football pitch.
“Hearing you sing my name has made me feel like the best defender in the world at times and the players will say without you this football club is nothing and I genuinely mean that.
“When I signed here as a 14-year-old I never dreamed of doing what I have done in the game. Let’s celebrate today. It is going to be sad and really emotional for me and my family.
“I love you from the bottom of my heart forever and will always be a Blue.”
